Guidelines for WCF assistance
In order to receive assistance from the WilliamChristopher Foundation you
should meet the following guidelines and submit all requested information:
-
Be a Medicaid recipient and hold a current Medicaid card

Be a quadriplegic with a C 5/6 level injury or above,
or an individual with cerebral palsy.

Have a Social Security card
-
Live in the United States Of America
-
Receive Supplemental Security Income (SSI) and/or
Social Security Disability Income (SSDI)
-
Complete and submit to the WCF an
application for
assistance
-
Submit a recent photo of yourself with the
completed application
